Emgality vs Nurtec for Migraine

Emgality is a monoclonal antibody that binds to CGRP and inhibits its effects. Nurtec ODT is a gepant medication that binds to and blocks the CGRP receptor. An orally disintegrating tablet, Nurtec ODT is taken by mouth while Emgality is given by a subcutaneous injection every month.

Migraine is a common headache/medical problem that is more frequent in women (25%) than men (6%). Migraine headaches with their intense headache pain, sensitivity to light and sound, throbbing, nausea and vomiting, and need to be down in bed are very disabling.

In a 3-month clinical trial between Emgality (galcanezumab) and Nurtec ODT (Rimegepant) Emgality was not statistically superior to Nurtec ODT in achieving a 50% or greater reduction in monthly migraine headache days.

 The clinical study.    

This was a 3-month double-blind study for 580 participants comparing Emgality with Nurtec ODT. The participants who were studied had episodic migraine. Placebo and true medication tablets and injections were employed in the study.

Medication dose.

The Emgality dose was 120 mg given as 4 subcutaneous injections. Four injections of Emgality were given as a loading dose of 2 injections (240 mg) followed by two monthly injections.

Nurtec OTC 75 mg was dosed as 1 oral tablet every other day.

Study results.

Although Emgality did better numerically than Nurtec ODT on endpoints it was not statistically superior to Nurtec ODT. Response rates for both of the CGRP antagonist drugs were similar.
